Nominations for Annual SDSHP Awards

Dear all,

SDSHP would like to begin soliciting nominations for our 3 annual awards.

1. SDSHP Pharmacist of the Year

2. SDSHP Technician of the Year 3. SDSHP Student Leadership Award & Scholarship.

Please see criteria below and send us an email at sdshp1@gmail.com with your nominations (and current CV if possible). The Nominations Committee will review applications and present the awards at the annual gala.

Pharmacist of the Year Criteria:

• Must be a SDSHP member

• Is or has been a professional mentor and/or role model to students, residents, and other pharmacists in or outside the workplace

• Is making or has made significant contributions to the profession in a contemporary and/or innovative practice setting;

• Has contributed to advancing the image of the profession of pharmacy in San Diego

• Has been actively involved in the organization at the regional and/or state level
  

Technician of the Year Criteria:

• Must be a SDSHP member

• Is making or has made significant contributions to the profession in a contemporary and/or innovative practice setting

• Has contributed to advancing the image of the profession of pharmacy in San Diego

• Has been actively involved in the organization at the regional and/or state level

SDSHP Student Leadership Award & Scholarship:

• The Student Leadership Award nomination form is attached and must be filled in it's entirety to be considered for nomination.

• The SDSHP Student Leadership Award & Scholarship recipient is a student who has made contributions to their school community, SDSHP, CSHP, and the profession of pharmacy. This can be demonstrated through, but not limited to, leadership roles, active involvement as a member of professional organizations, or exemplary leadership in promoting pharmacy to students and the community.
